We’ve created an affordable in-house dental program which is available to anyone who does NOT participate with an insurance plan. Our Rappold Family Dental Plan includes:
Semi-Annual Plan - $235
- 2 Prophylaxis Cleanings
- 2 Periodic Exams
- 1 set of Bitewing X-Rays
- 1 Fluoride treatment for children 14 yrs old, and under
- 25% discount off restorative services*
Quarterly Plan - $365
- 4 Periodontal Maintenances
- 2 Periodic Exams
- 1 set of Bitewing X-Rays
- 1 Fluoride Treatment
- 25% discount off restorative services*
Our Terms & Conditions:
- The Rappold Family Dental Plan is a pre-paid discount program offered only for services performed at Rappold Family Dentistry.
- This plan is not an insurance program.
- The enrollment fee is payable in full at the first appointment.
- The plan is good for 1 year from the purchase date and is non-refundable.
- All services must be completed within the enrollment year. Any work in progress will be provided with a one month grace period.
- Missed appointments will result in a $50 charge.
- Referrals will be provided for work not provided in our office. Our discount does not apply to these services or any other services provided outside the office.
* Our 25% discount cannot be combined with any other special offers. Limitations may apply to cosmetic and whitening services.
SENIOR DISCOUNT
15% off services, non-insured patients 65 and older, cannot be combined with any other offers.
